Please complete the form on this page to request your rebate. You will be asked to attach a photocopy of original dealer invoice(s) showing quantity sold and sales price. Each invoice must be on dealer letterhead with the MiraTEC trim purchase explicitly noted. Trim must be purchased between August 1, 2019 – October 31, 2019. No handwritten invoices will be accepted. Multiple invoices may be combined. Limit one swag bag per person. All requests must be received by JELD-WEN on or before November 31, 2019.
